DMC Employees (with DMC Care coverage) This searchable on-line physician directory includes DMC Care Physicians(Tier 1 - Primary Care and Specialists) only.  If you do not find your physician in the DMC Care on-line directory, your physician may not be a Tier 1 provider, but rather:                      Tier 2 Cofinity (previously PPOM),                  Tier 3 (Multiplan), or                  Tier 4 (Out-of-Network)      What are Provider Tier Levels? -- Click Here        If a DMC Care Provider refers you to another Provider or Facility for additional healthcare services, it is the Member's responsibility to verify that the referred Provider or Facility is Tier 1.  Selecting a Tier 1 Provider or Facility will ensure that the Member receives the maximum benefit coverage and lowest out-of-pocket expense.
