Welcome to your pharmacy benefits manager (PBM), CVS Caremark!

Your Prescription Card is mailed to your home address.  Be sure to take it to your pharmacy when you get a prescription filled for the first time. Use the ID number on the card to register at, where you can order refills, check drug cost and coverage, print a claim form and more.
If you have not received your Prescription Card, please contact:
CVS Caremark Customer Care at 1-855-559-8853 (toll-free).

Your plan sponsor chose CVS Caremark to manage your prescription care and associated costs. We offer these tips to help you save money on your prescriptions:

  1. Ask for generics first.  Generic drugs can cost up to 80 percent less than brand-name drugs.
  2. Remember the preferred drug list. If a generic drug is not available, ask your doctor to prescribe a drug on your plan's preferred drug list.
  3. Order 90-day supplies of long-term medications to save money. Sign up for DMC Mail Order to enjoy the convenience of prescriptions shipped directly to your home at no extra cost.
  4. Fill short-term prescriptions at a DMC On-site pharmacy or at a network pharmacy.  You will generally pay more for short-term (30 days or fewer) prescriptions that are filled outside the CVS Caremark Retail Pharmacy Network.

Call CVS Caremark Customer Care at 1-855-559-8853 if you have any questions regarding your prescription benefits or the CVS Caremark Retail Pharmacy Network.

DMC Care Customer Service is available, as well at 1-800-543-0161 to respond to your questions.

Have a healthy new year!
