Once completed, the HRA gives you recommendations for changes you may want to make toward a healthier lifestyle! Results are immediate! You can print down your results for reference. The HRA is completely confidential! |

| To take the HRA, follow the instructions below. Registration is required. For registration assistance, please call: (866) 860-3013 (choose option 1)
You also have the opportunity to speak with a Health Strategist once you have completed the HRA. Simply use the same number listed above. All information entered is completely confidential. Only you have access to your information.

Visit the website: www.dmcwellness.lifecoachconnect.com

Enter the User Name and Password that you found in the table provided (above).
Click on LOGIN. You will be recognized as not yet registered. Your Compay Name and Company Password will automatically display.
STEP 4
Fill out the your registration information (your profile) -- as required. Remember to write down your new User Name and Password. You will use this User Name and Password when you log in the next time you are ready to use the Life Coach Connect website. Your initial User Name and Password will no longer be valid.
STEP 5
Take the Health Risk Assessment (HRA). Click on the Health & Wellness tab, then click on Health Risk Manager to get started.
