Visit the Cofinity web-site at www.cofinity.net
Click on Search Now to find a Cofinity Network provider.
On the PPO Rental Networks web page, fill-in the Basic Search or Advanced Search page:

Check the box "I agree to Cofinity's terms of use".
Choose a location
Choose a provider type
Optional - specify a Doctor (Last Name) or Facility Name
Click on Search |

Advanced Search
Check the box "I agree to Cofinity's terms of use".
Choose a location
Choose "Group Health" as the Insurance Type
Choose a provider type
Optional - specify a Doctor (Last Name) or Facility Name
Choose one or more Specialty
Click on Search
