Effective 07/01/08 all prescriptions written for a 90-day supply of maintenance medications will only be available from the DMC Pharmacy Mail Service. DMC Care members continue to receive a 3 months supply for the cost of 2 months co-pay (a saving of one month's co-pay).
Remember, a 30 day supply of a prescription drug is always available at DMC On-Site Pharmacies.
DMC Care members, with a Michigan mailing address, may fill and refill prescription through the DMC Pharmacy Mail Service. The Mail Service order form is available at any DMC On-Site Pharmacy, Human Resource Forms Rack or, for faster service, use the online electronic form at www.dmc-care.org (click on the Pharmacy tab in the upper right hand drop down box and then on the Pharmacy Mail Service link). | |
