Chronic Disease

Chronic Disease is a long term illness:
  • May have multiple causes
  • The long-term effects may be hard to predict
  • May begin slowly and usually leads to more symptoms and loss of function
Chronic diseases -- such as diabetes, heart disease, asthma, chronic obstructive pulmonary disease, and cancer, are the leading cause of death and disability in the United States.

You can still live a healthy life and have a chronic disease.  A healthy way to live with a chronic illness is to work at overcoming the physical and emotional problems caused by the disease.

Asthma

Asthma is a predisposition to chronic inflammation of the lungs in which the airways (bronchi) are reversibly narrowed. Asthma affects 7% of the population of the United States and a total of 300 million worldwide

Cancer

Cancer is any malignant growth or tumor caused by abnormal and uncontrolled cell division; it may spread to other parts of the body through the lymphatic system or the blood stream

COPD

Chronic obstructive pulmonary disease (COPD) refers to chronic bronchitis and emphysema, a pair of two commonly co-existing diseases of the lungs in which the airways become narrowed. This leads to a limitation of the flow of air to and from the lungs causing shortness of breath

Diabetes

Diabetes is a condition in which a person has a high blood sugar (glucose) level as a result of the body either not producing enough insulin, or because body cells do not properly respond to the insulin that is produced

Heart Disease

There are many types of heart disease and it can affect the valves or the muscular structure of the heart. Some forms are a result of viruses or other diseases. Others are present at birth and some can be hereditary

 PATH (Personal Action Toward Health)

If you have a chronic disease or living with someone who does, DMC Care members can sign up for PATH (Personal Action Toward Health).
  • PATH is a 6 week workshop that helps participants improve their health and feel better.
  • Helps people cope with health problems
  • Improves management of symptoms
  • Two trained DMC Care leaders conduct the workshop (one or both may have a lifelong health condition)
PATH teaches you to set goals, break the symptom cycle, and understand how the way you eat and the way you exercise affect  your condition. The next PATH workshop will be starting September 15th from 5:30pm to 7:30pm for 6 consecutive Wednesdays at DRH. Registration is required.   Please call (313) 745-7822 or (313) 966-4365.
